Old Age Home Admission Application for SASSA Pensioners 2024- Step-by-step Guide

Your medical certificate would have a written statement that your mental health or body health is unfit and you need a permanent medical assistance, 24 hours a day for you. Agreed consent form and certificate by medical practitioner are required for legal assistance and issuance of court order.

SASSA application for old age home cannot be lodged online. Simply follow this step-by-step guide to apply for admission in old age home for SASSA pensioner.

  1. Visit nearest DSD office in your region or you can find your regional office of department of Social Development below.
  2. Fill out Old age home application and enter required information for the residential facility.
  3. Attach the following documents per required by old age home application form.
    • South African Identification card (ID card)
    • Valid Medical report
  4. Submit your completed documents and receive a stamped receipt of submission. 

Evidence of application

This stamped receipt is proof of evidence that you have applied and submitted the old age home SASSA pensioners application. If you would lost it, you can not claim reapplication on rejection of application.
